Why Progesterone Matters
For women who use estrogen therapy, progesterone plays a critical role by protecting the uterine lining (endometrium) from the stimulating effects of estrogen, reducing the risk of endometrial hyperplasia and cancer. However, not all progesterone products are the same.
Natural Progesterone vs. Synthetic Progestins
Micronized progesterone is bioidentical, meaning it is structurally identical to the progesterone naturally produced by a woman's ovaries. Because it has minimal activity at other hormone receptors, it is generally well tolerated.

Our Preferred Approach
When hormone therapy is appropriate, we generally favor bio-identical transdermal estradiol (patch, gel, or cream) because it avoids first-pass liver metabolism and has a lower risk of blood clots than oral estrogen.
We generally prefer oral micronized progesterone because of its favorable tolerability and beneficial effects on sleep. Progesterone appears to have a more favorable breast safety profile than synthetic progestins based on current observational evidence. It also builds bone density.
We do not routinely recommend medroxyprogesterone acetate (MPA) because evidence suggests a less favorable side-effect profile and, when combined with estrogen, an increased breast cancer risk observed in the Women's Health Initiative (WHI).
Breast Cancer Risk: What Research Suggests
The large French E3N observational study found that:
Estrogen combined with micronized progesterone was associated with little or no increase in breast cancer risk during several years of use.
Estrogen combined with synthetic progestins was associated with a higher breast cancer risk.
While no hormone therapy is completely risk-free, the type of progesterone used appears to matter.
