FDA Updates Menopause Hormone Therapy Warnings: What Women Need to Know

For years, many women have been hesitant to consider menopausal hormone therapy (MHT) because of warnings about breast cancer, heart disease, blood clots, and dementia. Updated FDA labeling is helping create a more accurate and individualized conversation about hormone therapy.

What Changed With Menopause Hormone Therapy?

The FDA has removed certain longstanding boxed warnings from menopausal hormone therapy labeling, including warnings related to cardiovascular disease, thromboembolism, breast cancer, and probable dementia.

These warnings were heavily influenced by the Women’s Health Initiative (WHI). Importantly, the women in that study were, on average, about 63 years old and many were considerably farther from menopause than women who typically seek treatment for menopausal symptoms.

Today, we understand that the benefits and risks of hormone therapy can vary depending on age, timing, type of hormone, route of administration, dose, and individual health history.

The updated labeling supports a more individualized approach rather than assuming all hormone therapy carries the same risk.

What About Vaginal Estrogen?

Low-dose vaginal estrogen is commonly used for vaginal dryness, irritation, painful intercourse, and certain urinary symptoms associated with menopause.

Because low-dose vaginal estrogen generally results in minimal systemic absorption, its risk profile differs from systemic hormone therapy.

The updated labeling may help reduce unnecessary fear surrounding local estrogen treatment. However, women with a history of estrogen-sensitive breast cancer should still discuss treatment with their healthcare and oncology teams.

Is Systemic Estrogen Safe?

Systemic hormone therapy can be highly effective for appropriate women experiencing hot flashes, night sweats, sleep disruption, and other bothersome menopausal symptoms.

However, the FDA changes do not mean hormone therapy is risk-free or appropriate for everyone.

The type and route of hormone therapy also matter. Oral and transdermal estrogen are not identical, and the choice may be important for certain women. For women with a uterus using systemic estrogen, progesterone is generally needed to protect the uterine lining.

The FDA's updated labeling represents an important step toward a more nuanced understanding of menopause hormone therapy.

It does not mean every woman should take hormones. Instead, it emphasizes the importance of evaluating each woman individually and making treatment decisions based on current evidence.
